Type 2 diabetes, er det blevet en folkesygdom?

Men hvad er diabetes 2, hvad forårsager det, og hvad kan du gøre, hvis du har lidelsen? Flere studier peger på, at det lille sorte morbær kan have indflydelse på forhøjet blodsukker og hjælpe til et vægttab. Begge dele har indflydelse på diabetes 2.

Det er en smule skræmmende at slå op på Diabetesforeningens hjemmeside og tjekke tallene for diabetes i Danmark. 1. januar 2016 var antallet af danskere med diabetes 260.750, hvilket svarer til 4,4 % af befolkningen. Type 2-diabetes tegner sig for ca. 80 %, hvoraf 30 % er udiagnosticerede. Der er altså tusindvis af danskere, som lider af diabetes uden at vide det.

Antallet af diabetespatienter er i stor stigning, og tallet er fordoblet fra 2000-2016. At antallet af danskere med type 2-diabetes kun vil stige støt, og der i 2030 vil være 430.000 danskere med lidelsen, er ikke mindre skræmmende.

 

Hvad er diabetes egentlig?

Du har sikkert hørt, at diabetes typisk deles op i to grupper, type 1 og type 2. Type 1 diagnosticeres typisk hos børn eller unge. Der er stadig uvished om, hvordan sygdommen udløses, og den kan derfor hverken forebygges eller helbredes. Insulinproduktionen i kroppen ophører fuldstændig, da bugspytkirtlens insulinproduktion er ødelagt. Patienten skal derfor regulere blodsukkeret med daglige insulinindsprøjtninger. Mere end 25.000 danskere lever i dag med type 1-diabetes.

Type 2-diabetes rammer primært voksne, men desværre er lidelsen også begyndt at husere hos børn og unge. Mindst 227.525 har type 2-diabetes og udgør dermed den største gruppe af diabetikere. I artiklen her har vi valgt at fokusere på type 2-diabetes, og hvordan blandt andet livsstilsændringer kan gøre en stor forskel.

 

Type 1-diabetes er en såkaldt autoimmun sygdom. Forskerne ved endnu ikke, hvad den udløses af. Sygdommen rammer oftest børn eller unge, men også ældre mennesker.

Type 2-diabetes er en kronisk sygdom, som tidligere blev kaldt gammelmandssukkersyge. I dag er det en misvisende betegnelse, da type 2-diabetes rammer både børn og voksne, tykke og tynde.

 

Har jeg mon diabetes 2?

Symptomer på diabetes 2 kan være svære at opdage. De udvikler sig langsomt, faktisk kan det ske over måneder og år. Symptomerne er i starten relativt svage og fejlfortolkes ofte som alderdomstegn. Du tager det måske som en almindelig skavank, som du for længst har accepteret og ikke har fået behandlet. Måske har du gået med diabetes 2 i årevis, uden du har tænkt videre over det. Pludselig en dag bliver sygdommen diagnosticeret ved et tilfælde, hvis du eksempelvis har været ved læge med noget andet ubehag.

 

Symptomer, du skal være særligt opmærksom på er:

  • Du føler oftere tørst
  • Du har hyppigere vandladninger
  • Du har større tendens til infektioner i huden og slimhinder
  • Du klør mere, især i skridtet
  • Du har synsforstyrrelser
  • Du heler langsomt, hvis du har fået et sår
  • Du er træt
  • Du har føleforstyrrelser
  • Du har ofte kvalme

Kan du krydse flere af punkterne af, så gå ikke i panik. Det behøver ikke at skyldes, at du har udviklet type 2-diabetes, men gå alligevel til lægen, og bliv tjekket. Bare for en sikkerheds skyld.

 

Type 2-diabetes diagnosticeres ved lægen ved en blodprøve, hvor det såkaldte ”langtidsblodsukker” måles.

 

Seks tegn på, at du er i risikozonen for at udvikle type 2-diabetes

  1. En BMI over 25
  2. Diagnosticeret diabetes under graviditet
  3. Et taljemål over 88 centimeter
  4. Diabetikere i den nærmeste familie (arvelighed)
  5. Mangel på motion
  6. Forhøjet blodtryk

 

Er du overvægtig?

Overvægt er blandt de største risikofaktorer for at udvikle type 2-diabetes, da tilstanden forringer insulinfølsomhed. Den forringede insulinfølsomhed betyder, at din krops celler ikke kan optage sukkeret fra maden, du spiser, og dermed give dig den energi, du har brug for. Sukkeret ophobes derimod i blodet og giver forhøjet blodsukker. Har du forhøjet blodsukker, så kan det på sigt føre til type 2-diabetes. Bugspytkirtlen slår alarm og arbejder hårdere og hårdere for at producere mere insulin, og overarbejde gennem længere tid gør den træt, og produktionen af insulin falder markant. Det forhøjede blodsukker skyldes, at sukkeret fortsat ikke kan komme ind i cellerne, men derimod flyder rundt i blodet, og ja, så får du forhøjet blodsukker.

Du kan altså med fordel prøve at tabe dig, både hvis du har diabetes, men også som forebyggelse af sygdommen.

Insulin er det stof, som hjælper sukkeret med at trænge ind i cellerne.

 

Fire ting, du kan gøre i forebyggelsen af type 2-diabetes:

  1. Motion. Vær fysisk aktiv mindst 30 minutter om dagen
  2. Kost. Spis sund og varieret kost, og undgå overvægt
  3. Alkohol. Begræns dit indtag af alkohol
  4. Rygning. Kvit cigaretterne, hvis du er ryger

 

Mål din talje! Måler du som kvinde over 88 cm eller som mand over 102 cm om taljen, er du i risikozonen for diabetes. Det farlige fedt sidder nemlig på maven.

 

Morbærs evne til regulering af blodsukker

Mulberry (hvid morbær; Morus alba L.) er i århundreder blevet anvendt i Japan som en infusionste. Mulberry-blade, -bark og -grene er længe blevet brugt i kinesisk medicin til behandling af feber, beskyttelse af leveren, forbedring af synet, styrkelse af leddene, til at lette urinering og til sænkelse af blodtrykket. Blade af morbærarter spises i Korea og Japan som ernæringsmæssige fødevarer mod for højt blodsukker til patienter med diabetes, fordi bladene indeholder 1-deoxynojirimycin (DNJ). DNJ er et stof, som er kendt for at være en af ​​de mest potente alfa-glycosidasehæmmere, hvilket hjælper med at holde blodsukkeret inden for et målområde ved at bremse fordøjelsen af komplekse kulhydrater (stivelse).

 

Morbær til vægttab

Ud over at virke regulerende på blodsukkeret kan morbær også hjælpe i et vægttab. I et omfattende studie opdagede et hold af italienske forskere, at morbær viser sig gavnlige ved et vægttab. Studiet blev udført med Mulberry tabletter fra New Nordic, og det viste, at personer, der tager hvid morbærekstrakt sammen med en kaloriereduceret diæt på 1.300 kcal taber sig tre gange mere end personer på diæten alene.

 

Kort om det randomiserede, placebokontrollerede studie

46 overvægtige eller svært overvægtige blev udvalgt og delt i to grupper. Alle blev sat på en identisk og streng diæt bestående af 1.300 kcal pr. dag. Den ene gruppe tog endvidere 2.400 mg morbærekstrakt, mens den anden gruppe fik placebo. Man fulgte op efter første, anden og tredje måned på grupperne med målinger af kropsvægt og taljemål, ligesom omkredsen af lår blev målt, og der blev taget blodprøver.

I gruppen, der tog hvid morbærekstrakt, var vægttabet efter tredje måned gennemsnitligt ni kilo svarende til 10 % af den oprindelige vægt. I placebogruppen var det gennemsnitlige vægttab på 3,2 kilo svarende til 3 % af den oprindelige vægt. Forskerne sammenlignede desuden blodprøver fra før behandling og efter behandlingen med hvid morbærekstrakt og så, at insulin- og glukosekurverne var lavere efter de tre måneder.

Blodsukkeret var hos begge grupper blevet mere stabilt, men mest markant hos gruppen, der fik hvid morbærekstrakt. Begge grupper havde mindsket deres taljemål og låromfang, men det var tydeligst hos dem, der havde tabt sig mest, altså hos morbærgruppen.

Derfor blev konklusionen fra forskernes side, at indtag af hvid morbærekstrakt er en fordelagtig understøttende behandling, når man går på kur. Ifølge studiet her tabte deltagerne, der fik hvid morbærekstrakt, sig tre gange så meget.

Det, forskerne kunne se i studiet, var, som vi også beskrev i overstående afsnit, at det bioaktive plantestof DNJ i det hvide morbærs ekstrakt blokerer alfa-glucosidasen og hydrolysen af polysakkarider. Alfa-glucosidase har til opgave at nedbryde stivelse og disaccharider til optageligt sukker.

 

Men hvordan virker morbær helt præcist?

I det medicinske tidsskrift Journal of Biological Regulators & Homeostatic Agents forklarer det italienske forskerteam kort, at vægt styres af vores gener og reguleres af vores hormoner, især insulin og glukagon. Samtidig opstiller de en hypotese om, hvorfor morbær øger vægttabet så markant hos folk på kur. Forskerne mener, at morbærekstraktet dels forhindrer blodsukkeret i at stige og dels forhindrer sukkeret i at blive lagret i kroppen som fedt. Ved indtag af kulhydrater vil blodsukkeret normalt stige, ligesom insulinniveauet og det glykæmiske indeks stiger. Men med morbærekstrakt blokeres omdannelse af komplekse sukkerarter til simple, og blodsukkeret forbliver således mere stabilt med lavere niveauer af insulin. Det lave insulinniveau er hensigtsmæssigt for et vægttab, idet insulin hæmmer frigørelsen af fedt fra fedtvævet, og du kan populært sagt kalde insulin et lægge-på-lager-hormon, men med lavere insulinniveauer kan fedtdepoterne tømmes. Det skyldes, at når kroppen ikke har sukker på lager (glykogendepoterne), vil kroppen i stedet gå over til at forbrænde fedt. Og fedtforbrænding resulterer, som du ved, i et vægttab.

Ved et dagligt indtag på 2,4 g morbærbladeekstrakt tabte testgruppen sig ca. tre gange så meget som dem, der udelukkende var på en kaloriefattig kost.

 

Mulberry fra New Nordic består af bladene fra den hvide morbær, men chrom er desuden også tilsat. Chrom hjælper til at forbedre transporten af glukose ind i cellerne, hvilket hjælper til at holde blodsukkeret stabilt.

Dosis er en tablet tre gange dagligt. Tag tabletterne et kvarter før måltiderne for at opnå den fulde effekt.
